Step-by-step solution
- Use Zoe's Lanyard to enter the clinic. Take the Scalpel from the green box in the Z Ellis room.
- Return to the pharmacy entrance and use the Scalpel on its door.
- Inspect the numbered posters and the CRTV broadcast before heading into the back room.
- Enter 6539 on the rusty locker.
- Take the Prescription Dosage Error note. It is one of the three items for the clinic scales.

If you are still outside the pharmacy, the locker combination will not open the entrance. You need the Scalpel from the clinic first.
